Built in 1904, this Queen Anne-style duplex at 409 Wisconsin Avenue, Madison, Wisconsin, features a clapboard and shingle-clad exterior with octagonal turrets at either end of the front facade, a complex hipped and side gable roof with octagonal projected sections over the turrets, a front gabled dormer with a gothic arched front projected gable and decorative paired brackets, one-over-one windows, two-story bay windows on the side facades, a front porch with a bonnet roof, paneled railing, heavy timber-style columns, and a clapboard-clad base, a front bay window in the center of the facade on the porch, and a rusticated stone base.

The house is a contributing structure in the Mansion Hill Historic District, listed on the National Register of Historic Places in 1997.
