Possibly not public domain. The National Library of Medicine says "Transmission or reproduction of protected items beyond that allowed by fair use as defined in the copyright laws requires the written permission of the copyright owners. Specific NLM Web sites containing protected information provide additional notification of conditions associated with its use."[1] If you look at the credit line, at [2], you can see that the photograph is credited to "Archives and Special Collections, Columbia University Health Sciences Division". To re-use this image you are advised to contact Columbia University direct.
