THE ASCENDING DOUCHE. 1058 This ordinarily consists of a jet or spray directed upward from a nozzle placed near the level of the floor (Fig. 42).It is chiefly useful for applications to the soles of the feet, theperineum, and the anal region. The ascending douche is usually applied at a very lowtemperature (60° to 45°), with a duration of half a minute toone minute. It is especially valuable in cases of atony of thebladder without dilatation, and in cases of weakness of thegenital organs not accompanied by irritation or pain. It isevident that in cases of chronic dilatation of the bladder thecold douche will prove of no value, as the muscular walls ofthe organ are so relaxed as to be unable to respond to thestimulus of the douche. The zvarm ascending douche is very useful as a means ofrelieving the pain of anal fissure and irritable rectum. The Jiot ascending douche is one of the most efficaciousremedies for the relief of pruritus ani ond pruritus vulvce.When employed for this purpose, the temperature shouldbe as high as can be borne. The alternate ascending douche is to be employed in casesof vesical paresis, chronic hypertrophy of the prostate, andexudates resulting from inflammation of the testicles or cord.
