This fever is variously termed urinary, urethral, and catheter, but its exact cause—whether septic or neurotic—it is impossible with our presentknowledge definitely to state. Certainly it occurs under thestrictest antiseptic precautions and with the utmost skill inpassing the catheter. The general treatment of this feveris similar to that following instrumentation or operation forstricture of the urethra, which has already been described, TREATMENT OF ENLARGED PROSTATE 91 except that, owing to the advanced age and debility of thepatient, it must be more sustaining, stimulants in moderationbeing allowed. When the urine contains pus, the local treat-ment will be the same as that presently to be described forcystitis. Cystitis. This, as we have already seen, is a common complicationof enlarged prostate, so that we must always be prepared todeal with it in its earliest stage. Fig. 36. odour, the best drug to administer is boric acid, which may begiven in 10-grain doses four times daily, A patient of mine,himself a medical man, who for years has been dependent onhis catheter, informed me that he found two or three largedoses of 25 grains each more effectual in bringing the urineback to its normal condition than repeated small doses; andI have since then frequently verified this experience in practice.If the urine becomes decidedly alkaline, the boric acid shouldbe combined with the benzoate of ammonia in 10-grain doses,or urotropine in doses of from 5 to 10 grains three times 92 TREATMENT OF ENLARGED PROSTATE daily may be given. This is particularly effective when theurine contains pus and mucus. When pus forms, the bladder must be washed out once ortwice daily with disinfectants or astringent lotions.
