English translation:

I, N. N., swear to God the almighty, that I shall not seek revenge for my imprisonment and punishment, neither from his Serene Highness the Lord of this territory, nor from his ducal house nor family, ministers, councellors, servants and subjects, nor their goods and chattles, in no manner or way, and that I shall not return to this territory ever again. So help me God and his holy word.
