Changes in the Tissues Surrounding the Nail, withor without Consecutive Lesions of the Latter (Par-onychia syphilitica).—In the more superficial variety ofthis disorder the epidermis and often the deeper portionsof the skin in a circumscribed patch, usually at one ex-tremity of the nail-groove, thicken and assume a wartyaspect. This local thickening may be resolved in suc-cessive exfoliations with some resulting tenderness, orthere may be superficial excoriations, fissures, or evenresulting ulcers. One or several digits may be involved,the fingers more often than the toes, on account of theexposure of the former in the occupations of life. Some-times the integument of one or several joints of the digitis implicated in the process. This complication occurswithin a few months after infection, or it may be delayedto one or two years after—rarely the latter. It is mostoften contemporaneous with maculo-papular and papulo- Plate 15.

In a deeper form of involvement of the tissues aboutthe nail, a nodule, dull ham-tinted and tender, varyingin size from that of a pea to that of a bean, forms eitherin the nail-fold, the nail-groove, or the matrix, usuallyupon one side. Occasionally the more prominent skin-symptom is a deep and ill-defined infiltration. Thecracking and exfoliation seen in the superficial formmay be conspicuous in the deeper form of the disease,the infiltration undergoing in favorable cases completeresolution under appropriate treatment, though its courseis commonly indolent. In other cases ulceration ensues,the part becomes tender, at times exceedingly painful,and the pus which may be discharged gives no suchrelief as in the run-around, the course of which is muchmore brief. The affection persistently lingers when thetoes are involv
